Any ideas as to the cause?

Guy claims he did nothing, just played NSMB, did not update (he says).

If you can boot the HBC install BootMii (if you haven&#039;t already )preferably as Boot2 but IOS should do. From there, you can then run <a href="http://filetrip.net/g25122545-Comex-Nand-Formatter.html" rel="nofollow">Comex&#039;s NAND Formatter</a>. All you need is the WAD of System Menu 3.2 and IOS30 I&#039;m pretty sure. Follow the Readme in the file and after that all runs, it should all be good. Well, at least it worked for me. I guess it&#039;s worth a shot.]]></description>

Waiting for required supplies to arrive to autoboot HBC. Recovery menu is functional.<br /><br />The owner choose to remove most system channels, now good bye maintenance mode. I suspect that the same error will occure during system menu install once he gets the HBC running.<br /><br />Any ideas about the cause/solution to crash/freeze during "Installing content #09..." sm install, 3.4 and 4.1 with dop-ios-mod and wad install.]]></description>
