I'm adding USB 2.0 support to OpenBOR, and I understand that to do so I have to reload IOS 202. What (if anything) would happen if reloading the IOS failed?

I believe if you try to reload to a non-existent IOS, the Wii will just produce a black screen and must be reset manually by holding down the power button.

You should check BEFORE reloading the IOS that it exists. I'm not sure how exactly you would do this, try looking at the source of something like SignCheck (if the source is open) or dop-Mii.
