This can be filed under SD card incompatibility. The fact that 4GB non-SDHC cards don't work is a known issue -- they're kinda weird -- but a lower priority than fixing SDHC cards. Your other issues are unrelated.by bushing - BootMii Beta

It is a bug in the installer -- has nothing to do with SD card. Unfortunately, the error code does not give us enough information, so we will add better error codes in the next version of the installer.by bushing - BootMii Beta
There are copyright issues that make it annoying to distribute a raw form of BootMii (since it normally reads boot2 from NAND, patches it, and writes it back out). However, it's pretty easy to install it with a hardware NAND flasher. boot1 is stored in block 0 of flash (pages 0-0x3f). 2 copies of boot2 (and one copy of BootMii) are stored in blocks 1-7. No, it can't intelligently move stuff around. Right now, if you try to restore a dump from one flash chip to another and the new chip has a bad block where the old chip stored a vital file, you're screwed. Most of the time, the bad blocks will fall in unimportant files, and when you boot, IOS can try to move around the data. Sometimes you're left with a system that can'tby bushing - BootMii Beta

I saw this once when trying to install using BannerBomb. It only seems to happen when installing BootMii/boot2 using BannerBomb; after installing boot2, it will say "SUCCESS" and then hang.
